- Unexplained infertility refers we don’t know why the couple is infertile.
- It is a limitation of our today s diagnostic medical knowledge.
- Infertility may be said to be unexplained if the woman is ovulating regularly has open fallopian tubes with no adhesions or endometriosis.
- If the man has normal sperm production & the post-coital test is positive.
- Intercourse must take place frequently particularly around the time of ovulation and the couple must have been trying to conceive for at least 1 year.
- 10% of all infertile couple have unexplained infertility.
Diagnosis –
- The diagnosis of unexplained infertility should consider only after all the tests have been performed & their result found to be normal.
- The frequency of this diagnosis will depend upon how many tests are done by the clinic. The fewer the tests the more frequent the diagnosis.
Possible cause of unexplained infertility –
Tubal Abnormalities
- It is possible that there may be a subtle defect in the mechanism by which the fimbria pick up the egg at ovulation.
- The cilia in the tube may not function properly.
- Tubal secretions may be improper or defective or not sperm friendly or not embryo friendly or embryo nutritive.
- The environment of the tube may not be optimum at the timing of ovulation.
Defective ovum
- Persistent production of the abnormal ovum.
- Ovum might be with the deformed structure or chromosomal abnormalities.
- Defective zona pellucida of ovum for acrosomal procedure.
- Directivity in respectively of an ovum with a particular structure of sperm.
Trapped eggs
- In some cases, eggs are produced & mature correctly within the follicle which then goes into becoming a corpus luteum without however first bursting to release the egg.
- The egg is therefore effectively trapped inside the unbroken corpus luteum called a luteinized unruptured follicle syndrome.
Luteal phase Abnormalities –
- Day of ovulation to day 1 is known as the luteal phase.
- It may be short duration or inadequate & this is called luteal phase defect.
- The hormone progesterone is essential for preparing the endometrium to receive the fertilized egg ( embryo ).
- The progesterone production rising is too slow or level might be very low may lead to defective endometrium and affect the endometrium receptivity.
- This condition is known as luteal phase abnormalities.
- Perfect diagnosis of lutead phase abnormalities is not possible.
Immunological factors –
- The Immune system can react against the man’s sperm & kill them immobilize them or make them stick together.
- Women can also develop an immune reaction to the coating of their ovum eggs which can prevent fertilization of the ovum.
- Women s immune system can prevent her embryos from implanting in the uterus.
Infection –
- Certain undetectable infection of mycoplasma or chlamydia may be present in person may lead to unexplained infertility.
The inability of sperm to penetrate eggs & defective CS 1 protein
- Some men have complete normal sperm count with good motility but sperm is defective with an acrosomal enzyme can not give Acrosomal reaction with the ovum.
- Sperm defective in CS 1 protien (clevange signel 1 protien)is not started clevange cell divison after fertilization.
- Strong CS 1 protein is necessary for initiation of cell division & continuity of development of the embryo.
- IVF goes fail in case of CS 1 protein defective sperm, even after successful ICSI.
Uterine factors
- Some defective intrauterine protein is also responsible .
- Some women have an abnormal endometrium which does not allow the embryo to implant, in this kind of case IVF goes to fail. It is difficult to diagnosis.
- In some unfertile women, the endometrium remains persistently thin.
- This may be because of inadequate uterine blood flow or poor estrogen receptors in the endometrial cells.
Psychological factors –
- Stress & anxiety produce more production of cortisol hormones & it leads to defective or low-quality ovulation.
- So stress & anxiety might because of infertility in many cases.
Improper sexual intercourse –
- Sexual intercourse at long intervals associated with weak and less motile sperm in semen. This condition is also responsible for unexplained infertility.
- The regular sexual activity is necessary.
Defective cervical mucus –
- Cervical mucus is much more important to improve the capacity of sperm & easy passage of sperm from the vagina to uterus.
